So how does this work ? You just buy one, pop your SIM card inside and it just works, or you need to pay extra to activate all the functions that go beyond the 'basic phone' setup ?

More or less, yes. Once you leave the GSM world and SIM cards, it becomes much harder to move phones from one carrier to another. For example, Sprint will generally only activate phones it sells you -- you can't get a cheapie from a sprint-reselling MVNO and then take it to sprint.
